Case management is a huge part of the recovery process, which is why Agape Wellness Centers has partnered with Gateway Community Outreach. We understand that continuity of care, accessibility, accountability and efficiency is important for case management and we strive to make services more accessible and barrier free to all our clients.
Gateway assists over 30,854 clients each year with a staff of 4 and nearly 300 volunteers, through 8 satellite locations and provide food and emergency assistance to prevent homeless through the utilization of their EFSP and Foundation grants.
Their success is contributed to the utilization of their case management system, consisting of a 90-day plan to connect clients with services and referrals (i.e.) food, bus passes, baby needs, gift cards for clothing, rent/mortgage and water bill assistance.
Their mission is to safeguard and protect our most vulnerable community members by providing food, housing assistance, referrals and intensive case management to those in need of emergency aid.
